Resident Feedback

It’s our goal to constantly improve the resident and family experience.  If you have either verbal or written feedback for us, including suggestions, recommendations, concerns, grievances of any type, or compliments for any individuals, groups, or the community as a whole, we want to know.

If you wish to fill out a form in person, written forms are available from our front desk receptionists and throughout the buildings in black feedback boxes.

Completed forms can be returned in the slot of any of the black boxes, or mailed to us at:

The form may be filled out below, or also be given to any social services staff member. Alternatively, you may also email us (without a form) at [email protected]. Verbal feedback can be given to a social services team member or by contacting the nurse manager on duty after hours and on weekends.  Feedback can be given with your contact information or anonymously (anonymous feedback should be provided verbally or via email).

If any allegations need to be included, please call your loved one’s social worker or nurse so they can communicate it through our reporting line.  Allegations are taken seriously and need to be addressed quickly.
