MDS RN ASSESSMENT NURSE – Full Time

GENERAL SUMMARY:

Under the direction of the Director of Nursing (DON) and the MDS Director is responsible for assessing residents/patients according to Resident Assessment Instrument. Is responsible for the development of care plans upon admission and maintenance of care plans throughout the resident’s/patient’s stay.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Coordinates and completes the RAI in compliance with state and federal regulations and Facility policies and procedures.
  2. Performs administrative responsibilities associated with resident/patient care as related to
    1. Medicare Perspective Payment Schedule
    2. MDS and CAA Procedure
    3. Care Planning and Implementation of the Plan
    4. Evaluation of the Care Planning Process
  3. Determines need for and conducts change of condition assessment.
  4. Responsible for the nursing resident/patient care plan which includes writing the care plan, communicating with staff on the unit regarding the plan, coordinating and communicating with other disciplines, and updating the plan and data sheet as needed daily.
  5. Participates in admission, condition change, quarterly care conferences, and discharge planning involving the IDT, resident/patient and/or family.
  6. Implements necessary documentation pertaining to assessments and care plans.
    1. Pain assessments
    2. Falls
    3. Bowel and bladder assessment
    4. Quarterly summaries
    5. Side Rail Assessment
    6. AIMS
    7. Dehydration Assessment
    8. Braden Scale
  7. Coordinates the assessment activities of the Interdisciplinary Team.
  8. Schedules and conducts Interdisciplinary Team Meetings to assure completion of care plans as indicated by the Resident Assessment Protocols. Monitors care plans to ensure they are being followed. Reports non-compliance to the MDS Director who reports to the Director of Nursing.
  9. Keeps 802 updated for their residents.
  10. Ensures completion of Medicare Certification by physicians as directed by the MDS Director.
  11. Performs admissions as directed by the Unit Manager and ensures care plans are initiated and all paperwork is in order including orders for both medications and treatments.
  12. Completes discharge paperwork and calls in medications to Pharmacy of choice.
  13. Attends and provides input in meetings held by clinical and interdisciplinary team.
  14. May perform any duties of a RN Staff Nurse.
  15. In event of absence of the Unit Manager, will assist in follow-ups concerning resident/patient care.
  16. All other duties as assigned.

UNIT MANAGER (MIDNIGHT SHIFT)

GENERAL SUMMARY

Under the direction of the Director of Nursing and/or the Assistant Director of Nursing, assists in the Nursing Department program planning, goal setting and policy formulation. Directs the operation of the Nursing Units and disciplines, evaluates and supervises nursing personnel.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  1. Responsible for operation of Nursing Unit(s), on both LTC and Rehab, including leveling of staff, assigning resident groupings and supervision of all nursing personnel. Reviews and adjusts resident groupings when necessary to ensure appropriate work load assignments.
  2. Participates and recommends discipline when appropriate and is responsible for timely evaluation of all staff within assigned unit(s).
  3. Assists in the orientation and training of all newly hired nursing staff. Observes newly hired employees after training to ensure that resident care needs are being met and makes appropriate referrals to the Education Coordinator if resident care needs are not being met.
  4. Monitors charting of nurses and CNAs for completeness, appropriateness and timeliness.
  5. Communicates with families, physicians, Nursing Office and other Facility departments regarding resident status and unit needs.
  6. Communicates with team leaders and CNAs on other shifts regarding resident, family and Facility concerns, needs and changes in procedures and/or expectations.
  7. Completes Continuous Quality Improvement (CQI) Audits as directed by Director of Nursing.
  8. May be placed on call at the discretion of the Director of Nursing.
  9. Makes rounds to evaluate effectiveness of care delivery and performance of nursing staff to ensure compliance with CQI standards.
  10. Organize and keep get-up list updated; coordinate with the Unit Manager on day shift.
  11. Responsible for updating all profiles, TARS, and MARS.
  12. Check calendar for lab work and lab slips.
  13. Check if any residents need to be ready for early morning physician appointments, special events, and/or surgeries.
  14. Fill out Census Board and ensure complete.
  15. Oversee that chart checks are being done.
  16. Oversee cleaning of refrigerators, med carts, tx carts every month.
  17. Check glucometers and calibrate as needed.
  18. Follow up on care conference concerns specific to your shift.
  19. May perform any of the duties of the staff nurse.
  20. All other duties as assigned.
